University Certificates
of Insurance

Overview

The University is often asked to provide proof of insurance for agreements, contracts, internships, events, facility use, and vehicle rentals. The Office of Risk Management provides this in one of two ways:

Insurance Verification Letter- Our standard proof of insurance for most agreements. The letter confirms LSU's coverages and limits and can be tailored to the agreement.

Certificate of Insurance- Issued by the University's insurance broker when the other party specifically requires a certificate.

We will determine which document fits your request.

Additional Insured and Waiver of Subrogation- As a Louisiana public entity, LSU typically cannot provide additional insured status or agree to waivers of subrogation, as the Louisiana Constitution and state law restrict the University from indemnifying or releasing recovery rights except in limited circumstances authorized by law. If the other party is requesting either, note it on the form and we will evaluate the request.

To request proof of insurance, complete the form below and attach the agreement or the other party's insurance requirements. Please allow 10 business days for processing.

If immediate assistance is needed, please email the Office of Risk Management.
